You qualify for Access to Care if you:
- Live in SUBURBAN Cook County or northwest Chicago (west of Pulaski Road AND north of North Avenue).
- Have a family income less than 300% of the federal poverty guidelines.
- Do not have health insurance (or a deductible of $1,500 or more per person) and ineligible for Medicare*, Medicaid (including All Kids, FamilyCare or CountyCare). *The program will cover individuals on Medicare that are unable to afford Part B and meet the other eligibility requirements.

There is a one time Enrollment Fee:
$20 for one person;
$40 for two people;
$50 for three or more Adult Members in one family. (This applies to husband, wife, and dependent children age 19 to 26 ONLY.)
Our program provides: visits to your assigned Access to Care doctor for a $5 co-pay. Basic Lab and X-ray services for $5 per draw or procedure, and prescription medication for $15 (generic), $30 (preferred brand) and $40 (non-preferred brand).
